This survey is a simple
Microsoft FrontPage FORM and isn't very smart.
For the three "fill in the blank" questions, after typing your
answer DO NOT PRESS "ENTER". Instead, press the "TAB" key to
move to the next item.
I read all instructions and
responded to all items -- this took 17 minutes

I did NOT access any reference material.
Remember, you must go to the
bottom of the survey and click "SUBMIT" for your results to be
tabulated and sent to me.
